I agree and acknowledge that I have voluntarily chosen to participate in physical exercise at Energy Fitness.  I acknowledge that I have been informed of the potential dangers of physical exercise including, but not limited to, abnormal blood pressure, fainting , heart attack, or death.  I assume all responsibility and risk for my health and well being and hold harmless M A J Fitness LLC, dba Energy Fitness, including owners, partners, officers, agents, instructors, trainers, and employees from all damages or claims directly or indirectly resulting from my participation in a membership, including any group fitness classes or bootcamp classes at Energy Fitness.

This membership is valid for only 2 people.  If this membership is used by anyone else, it will be terminated without refund.

This gym is under 24 hour surveillance.  To bring a guest without the approval of management will be grounds for termination without refund.   

A shirt must be worn at all times, and no open toed shoes are allowed.

No excessive yelling or vulgar language will be tolerated. Members must no abuse or damage equipment no dropping or throwing weights).

Any membership may be cancelled at the sole discretion of management at any time.

We reserve the right to refuse service to anyone at anytime.

We reserve the right to cancel any programs at anytime.

No one under the age of 15 is allowed inside the gym.

After the trial ends, 7 day notice is required to cancel memberships.
